 Services Provided


PEER SUPPORT SERVICES

Designed for individuals ages 18 and older with mental health and or substance are addiction service needs to support recovery.

  • Writing Care And Recovery Plan
  • Advocacy (making sure your voice is heard)
  • Accessing Mental And Substance Abuse Services
  • Strengthening Coping, Decision Making, And Other Skills
  • Developing Relationships  With Caring People In The Community
  • Locating Housing, Education, Employment, Meals And Social Activities

Day Support

Program Services are provided to increase functional abilities in areas such as communication, improvement in self help, socialization skills, personal care, and functional academics.


Respite

Respite is a service that provides periodic relief for the family or primary caregiver.  We provide individual services which may include in and out-of-home services, activities in a variety of community locations, and may include overnight services.  This service enables the primary caregiver(s) to meet or participate in scheduled and unscheduled events and to have time away from the child.


Individual Support Services

Designed for individuals ages 18 and older with severe mental health needs who are moving to and living in independent housing in the community.  This service helps adults live more independently in the community.

Examples include assistance with:

  • Housing
  • Preparing Meals
  • Using The Phone
  • Laundry
  • Housekeeping
  • Shopping For Household Necessities
  • Money Management
  • Medication Management
  • Transporting To Access Community





Personal Assistance

Service provides aid to person to allow them the opportunity to engage in activities and interactions from which the person would otherwise be limited or excluded because of disability or disabilities. The assistance includes:  (1.) assistance in personal or regular living activities in the client's home; (2) support in skill development; or (3.) support and accompaniment of the client in regular community activities or in specialized treatment, habilitation or rehabilitation service programs.


Community Guide

Service provides support to individuals and the teams that assist individuals in developing social networks and connections within local communities.  The purpose of this service is to promote self-determination, increase independence, and enhance the individual's ability to interact with and contribute to their local community.

Community Living And Support
Community Living and Support provides individualize services that enables the 
individual to live successfully in his/her own home; helping them to learn new skills, practice and or improve skills in areas that directly affect their ability to reside as independently as possible.  This may include interpersonal, independent living, community living, self-care and self determination.
